This month, the Virginia Center for Health Innovation was awarded a one year grant from the Potomac Health Foundation (PHF) for the creation of the Potomac Primary Care Collaborative (PPCC). The PPCC will engage up to 20 primary care sites in a regional learning collaborative aimed at strengthening practice models, optimizing clinical care, and enhancing clinical-community linkages to create a stronger system of care. Recruitment of primary care sites will begin in late summer/early fall.
